THE team behind a model to help provide the right type of diabetic care has been short-listed for a national award.
Called the ‘super six’ model, it was devised by staff working at both Portsmouth Hospitals NHS Trust and Southern Health Foundation Trust.
Now they have been short-listed in the Managing Long Term Care category at the Health Service Journal Awards.
The diabetes team at Queen Alexandra Hospital, Cosham, say they are pleased to be recognised.
Diabetes consultant Partha Kar said: ‘By identifying the super six model, delivery of diabetes care has been considerably improved.
‘The new model provides greater clarity and support for GPs, identifying six specific categories of diabetes, which require specialist expertise that could not be provided in the community.
‘It’s great to see this work recognised, as we have all worked extremely hard to ensure its successful delivery and improve care for the benefit of patients.’
The winners of the awards will be announced on November 20 at a ceremony in London.
