A COMMUNITY diabetes team is up for another award.

The ‘super-six’ model of diabetes care has been shortlisted in the Guardian newspaper’s Healthcare Innovation Awards.

The model, which is run by Portsmouth Hospitals NHS Trust, with South Central Ambulance Service, has been nominated for reducing hospital admissions through a six-step pathway.

Partha Kar, diabetes consultant, said: ‘We were the only team to submit plans for the pioneering seven-day working week to cover diabetes care.

‘I want to thank the whole diabetes team as without their hard work, this could not have been achieved for our patients.’

The awards take place on Thursday.
