New scheme to get QA Hospital patients home soonerÂ

Home First is a new approach for the Cosham site which will get staff asking themselves '˜Why not home? Why not today?'.

It is hoped the project will increase the number of patient discharges.

Sarah Eggleton, director of integrated care, said: '˜Home First aims to discharge patients in a prompt, timely way, to their own homes wherever possible.

'˜Home First requires minimal assessment in an acute hospital bed, and onwards assessment in a community setting, with the right support and care.'

The programme was rolled out last week on four wards at QA Hospital.
