Fareham MP Suella Fernandes was appointed to chair the question and answer debate with Southern Health boss Katrina Percy.

It comes after the CQC issued a warning to the trust ‘to improve its governance arrangements to ensure robust investigation and learning from incidents and deaths, to reduce future risks to patients.’ It followed a CQC inspection held in response to a report exposing flaws in the investigation of deaths.
